We are a non-profit charity, providing hospice and community services to the people of New Tecumseth and surrounding areas supporting living life fully and dying with dignity. We walk together with families and community members through the journey of illness and grief offering a variety of day programs, counselling, and palliative care. We believe everyone should have access to the right palliative and bereavement care when and where they need it. We are excited to be developing plans to expand our services to the community in the near future.
SCOPE: Reporting to the Manager of Integrated Care Team, the Registered Practical Nurse ensures that the Memory Care and Dementia Day Program clients are provided with ethical care, with patient and family centred focus.
KEY RESPONSIBILITIES:
Assumes responsibility for client care in accordance with the Regulated Health Professionals Act, and CNO. Provides direct nursing care and support to complete care as applicable in consultation with the manager and the team.
DUTIES AND RESPONSIBILITIES:
Schedule and conduct day program assessments and collaboratively develop care plans/goals for each participant.
Review and update participant care plans on a regular basis.
Maintain program resources and equipment and purchase program supplies and materials relevant to participant needs.
Support and train other individuals, such as volunteers and other day program staff, in planning and implementing the activities.
Assist with other coordination activities for the program such as transportation, community referrals and administrative tasks, as needed.
Support participants with their ADLs including transfers, toileting, changing soiled clothing and medication administration.
Assist kitchen staff with determining appropriate meals for participants based on their dietary needs and restrictions.
Proven clinical experience providing holistic patient/family care, including strong patient assessment, care planning and implementation, patient education, psycho/social support, with excellent documentation skills.
Demonstrated commitment to quality care and safety with an ability to exercise sound professional judgment and use best practice guidelines and evidence-based practice.
Ability to work effectively independently and collaboratively within an interprofessional team.
Addressing and resolving issues in a productive, respectful manner.
Contributing to a safe, inclusive environment for all, through compliance with patient and staff safety policies and procedures.
Excellent interpersonal and listening skills with an ability to work effectively with others including patients and families.
Demonstrated strong decision making, problem solving and critical thinking skills.
Demonstrated commitment to continuous learning and professional development.
Computer proficiency with experience working with electronic medical records.
Practices in accordance with the CNO’s Confidentiality and Privacy-Personal Health Information Practice Standard and adheres to the policies and procedures of Matthews House Hospice.
